1, Health-Perception-Health-Management Pattern

The client describes herself as a healthy individual. She is healthy but she sleeps
late at night, and doesn’t follow the correct sleeping hours. She sleeps at 3 or 5 o’ clock
in the morning and she wakes up at around 7 o’ clock AM.

2. Nutritional-Metabolic Pattern

The patient doesn’t like to drink too much water, she likes sodas more than
water. She drinks about 2-3 glasses of water everyday but drinks about 2 liters of sodas
everyday. She eats oatmeal, bread and jam in the morning, she eats a heavy meal for
lunch, takes a snack in the afternoon and salad in the evening.

3. Elimination Pattern

Patient goes to the bathroom about 6-7 times a day and characterized it as clear
and yellow. She also defecates thrice a day. She describes the character of her stool as
brownish in color and semi-solid.

4. Activity-Exercise Pattern

The patient doesn’t exercise much, it is because for her it is tiring and a waste of
time.

5. Sleep-Rest Pattern

The patient doesn’t sleep early, she doesn’t follow the correct sleeping hours
which is 8 to 10 per 24 hours. She usually sleeps at around 3-5 in the morning and
wakes up at around 7 o’ clock am.
6. Cognitive-Perceptual Pattern

The patient is oriented, she is able to focus and attentive. She was able to
answer the questions properly.

7. Self-Perception-Self Concept Pattern

The patient is a lively and energetic person, she likes to go outside with her
friends and have fun. Since she is in her teenage era it is normal for her to be energetic
and a happy person.

8. Role Relationship Pattern

The patient is a reliable friend, she is also an understanding person to her family
and friends. She thinks about others more than herself, she is a selfless person.

9. Sexuality-Reproductive Pattern

The patient doesn’t have any diseases or problems in her reproductive


system/organ. Furthermore, she is satisfied with her sexuality.

10. Coping-Stress Tolerance Pattern

The patient usually spends 8-9 hours on her gadgets to distract herself during the
day.

11. Value-Belief Pattern

The patient’s beliefs affect her way of living. For instance, going to church every
Sunday to pray and thank/praise God.
