    AFFIDAVIT OF PARENTAL CARE AND CUSTODY

    I, _________________________, Filipino, of legal age, with residence address at

    accordance with law, hereby depose and say:

    That I am the mother and legal guardian of the minor/s, _________________________________________,


    where I exercise parental authority over my said child/ren supporting his/her daily expenses

    That the total value of the aforementioned property child including the present value of Life Insurance Policy
    No. ________________________________________ which he/she has an interest or to which h/she has a claim,
    does not exceed Php 500,000.00.

    That as the parent of the said minors, I am the legal administrator of his/her property; that I have not been
    disqualified by any competent authority, nor I am suffering from any legal disqualification to administer the
    property of the said children.

    That I file this affidavit with the BDO Life Assurance Company, Inc with the request that I be allowed to
    represent the aforementioned children in claiming benefit under Policy no. ________________ issued on the life

    That in consideration of the action the Company in granting my aforesaid request and in allowing me to
    receive the aforesaid proceeds of any other benefit in behalf BDO Life Assurance, from any and all consequences
    of said action and warrant to defend forever said action against any and all persons who may assert right under the
    policy or file and adverse claim thereon and to REIMBURSE the Company of whatever payment it may mamke,
    or damages and expenses it may occur, by reason of such action.
