JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS


RE: ONE AND THE SAME PERSON

We, ROEL A. MARBELLA, of legal age, Filipino and with residence address at No. 44 F.
Caparas St., IVC, Marikina, and MARCIANA G. PORLAJE, likewise of legal age, Filipino and with
residence address at 62 F. Caparas St., IVC, Marikina City, respectively, after having duly sworn to in
accordance with law do hereby depose and say, that:

1. We personally know MARIA ARCELE CAYABYAB TORRALBA (a bona fide member of Social


Security System ), who is our long time neighbor and acquaintance at Caparas St., Olandes IVC, Marikina
City, and the fact of her birth is registered at the Local Civil Registrar of Quezon City.

2. That she is the daughter of Spouses PRIMITIVO LAMBINO TORRALBA and JULIANA NICOLAS
CAYABYAB (both deceased) and was born on March 20, 1961 in Quezon City;

3. That she was married to RODOLFO GIGANTE BAMBA (deceased), hence, since their marriage
union, she has been using MARIA ARCELE TORRALBA BAMBA in all her government issued IDs ID such as
SSS ID and Senior Citizen’s ID.

4. However, in her data in Social Security System (SSS), as per SSS Form RS-1 with SS Nr. 03-
6198130-5 her name reflects as MA. ARCELE TORRALBA BAMBA, while in SSS Form RS-1 with SS Nr. 02-
3175047-5, her name MA. ARCELI TORRALBA BAMBA; on the other hand, per SSS Web Inquiry System
with SS Nr. 03-6198130-05, her name reflects MARIA ARCELE TORRALBA BAMBA which refer to one and
the same person;

5. Considering that she is now a widow, she is desirous to amend her name MARIA ARCELE
TORRALBA BAMBA, and be presently known as MARIA ARCELE CAYABYAB TORRALBA, and the latter be
reflected in all her present undertakings, in order to remove further confusion in the future as to her
correct name.

6. That neither of us is related to any of the foregoing persons within the fourth civil degree
either by consanguinity or affinity, nor do we have any pecuniary interest in this matter whatsoever.

7. That we joint executing affidavit to attest to the truth of the foregoing instrument and for any
legal purposes this may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this 4 th day of June, 2021 in Quezon
City.
