I am XYLA JOY A. PEREZ, single, an IPED teacher I at Sikat Mactan Elementary


School, Sitio Mactan,Kasapa I, Loreto, Agusan del Sur. I was hired last November 15,
2016.

I heard that our Department of Education, Division of Agusan del Sur would release new
items for teachers. I am longing and I would like to grab this opportunity to request for
transfer to SAN ISIDRO NATIONAL HIGH SCHOOL , San Isidro, Loreto, Agusan del
Sur. I am currently residing at Poblacion, Loreto, Agusan Del Sur.

I have this intent due to following reasons:


1. The area of my assignment was too risky to reach; I have to cross a lake during
flood and to walk muddy areas during the dry season;
2. That it is my greatest opportunity because my present station is far from my
residence hence time and travel expenses could be of great help;
3. That I am a bona fide resident of Poblacion, Loreto, Agusan del Sur near to my
desired station and I am pretty sure that I can extend many services even
beyond official hours for the benefits of the children and a safer travel going
home;
4. Being a breadwinner of the family, I have my mother and father that really need
my support, attention, and care.
