The nursing assessment identified disruption of the skin surface and painful skin lesions. The nursing diagnosis is impaired skin integrity. The goals are for the client to have reduced risk of further impairment and experience healing of lesions within 6-8 hours. Nursing interventions include inspecting the skin daily, applying appropriate dressings, and supplementing the diet. The expected outcome is healing of lesions and regained skin integrity with reduced infection risk within 3-4 days.
