This AGREEMENT is entered into this ___________________, at the City of Davao, by and
between:
-AND-
SALMAN P. JOHAYR, RMT, Filipino, of legal age, single, and with postal address at
Davao City hereinafter referred to as the EMPLOYEE
WITNESSETH:
WHEREAS, the EMPLOYER hereby offers and EMPLOYEE hereby accepts probationary
employment in the following rank-and-file position: MEDICAL TECHNOLOGIST-STAFF
2.1 Collect blood or tissue samples from patients, observing principles of aseptic
technique to obtain blood sample.
2.2 Delivers test result to physicians or patients.
2.3 Prepares standard solutions and reagents to be combined with samples,
following standardized formulas or experimental procedures.
2.4 Perform related work as required.
3. EVALUATION
The standards upon the EMPLOYEE will be evaluated on has likewise been
explained to the EMPLOYEE by the EMPLOYER at the time of engagement.
4. REGULARIZATION
By or before the end of the Six (6) month period, the EMPLOYEE may acquire
regular status upon evaluation and recommendation of the immediate manager.
Upon regularization, the EMPLOYEE shall be entitled to all company benefits and
privileges enjoyed by all regular employees of similar rank and tenure.
5. COMPENSATION
The EMPLOYEE shall be paid a gross rate of FIVE HUNDRED PESOS (P500.00)
per day, subject to government-mandated deductions to be borne by the
employee, every 15th and 30th/31st day of the month.
The EMPLOYER may, as its discretion, grant bonuses, allowances, or benefirs not
defined in this contract. Such exercise of discretion shall not be considered as
established practice or precedent and shall not be demandable under this
Employment Agreement or any other written or unwritten agreement.
6. WORK HOURS
The EMPLOYEE may be required to work more than her regular work schedule
due to the demands of the business or the requirements of the EMPLOYER.
7. PLACE OF WORK
The EMPLOYEE shall follow all company rules and regulations, written or
otherwise. All existing and future company rules and regulations shall be
deemed incorporated in this AGREEMENT.
9. TERMINATION OF EMPLOYMENT
The EMPLOYEE shall give a written notice of their intention to resign from their
employment at least thirty (30) days prior to the effective date of their
resignation.
11. NON-DISCLOSURE
The EMPLOYEE shall not divulge to any third party any part of this AGREEMENT,
including salaries and other remunerations unless legally required by law,
statute, or any competent authority.
12. CONFIDENTIALITY
The EMPLOYEE understands and acknowledges that all records, documents, and
other information, written or otherwise, are confidential and shall not be
disclosed during and after employment with the EMPLOYER.
This AGREEMENT represents the entire agreement between the EMPLOYER and
EMPLOYEE and supersedes all prior negotiations, representations, agreements,
either oral or written.
IN WITNESS WHEREOF, the parties have hereunto affixed their signatures on the date
and place first stated above.
ACKNOWLEDGMENT
All known to me and to me known to be the same persons who executed the foregoing
EMPLOYMENT AGREEMENT and they acknowledged to me that the same is their free
and voluntary act and deed.
IN TESTIMONY WHEREOF, I have hereunto set my hand and affixed my notarial seal on
the date at the place written above.
