Employee Referral Form - Chila

The employee referral form provides guidelines for employees to refer potential applicants for open positions. Employees are eligible for a referral bonus if they refer an external applicant who is successfully employed for 30 days. Only one referral is allowed per employee. The referring employee must agree to have their name used during the hiring process. The form collects information about the referring employee, referred applicant, and the applicant's skills and relationship to the referring employee. HR will use the information and may interview the applicant to decide if they are hired.

Referral Guidelines

1. To refer a potential applicant, you need to complete this form. Send to [email protected] once done.
2. You are eligible for a referral award only when you refer external applicants.
3. All referral bonus payments will be paid to the employee after the applicant successfully completes 30 days
of employment.
4. Employees involved in the hiring decision for a particular position are not eligible for referral awards.
5. Only one referral will be allowed per employee. If you have more than 1, Please rank each applicants
in order of importance with #1 being the most qualified applicant and #3 being the least qualified applicant.
6. The referring employee must agree to have his or her name used when QuikfillRx contacts the applicant.
