COMES NOW, Applicant-movant thru counsel, respectfully alleges that:

1. That Applicant-movant has a pending application for issuance of Certificate of


Public Convenience to operate Transportation Network Vehicle Service (TNVS)
within METRO MANILA accessible to motor vehicle traffic and vice versa with the
use of ONE (1) unit/s and in consideration thereof, she/he was issued a
PROVISIONAL AUTHORITY to operate said service.

2. That the FORTY-FIVE (45) days authority granted has already been expiring
whereby applicant-movant hereby request for another EXTENSION of validity of
provisional authority for the operation of the following applied unit.

4. That applicant-movant will comply with comply with all the requirements of this
Honorable Board and will abide with the existing rules and regulations, the Public
Service Act and other pertinent law/policies.

5. That will the consideration and approval of this Motion for Extension of validity of
provisional authority to operate the service, public necessity and convenience will
be promoted in suitable manner.

WHEREFORE, premises considered, applicant respectfully prayed to this Honorable
