The Company is pleased to inform you that your employment with this institution, as Helper, is now
regularized effective August 1, 2019. Thus, you will enjoy the benefits that this institution is providing.
1. Your daily wage is 538.46 pesos; you are subject to no-work-no-pay policy.
2. You shall be paid twice a month on the 15th day and on the last working day of the month.
3. You are entitled to meal accommodations during working days.
4. Any bonus or declarations of bonus shall be made at the sole and absolute discretion of the
community of the Company. In determining the amount of bonus, your work performance shall be
considered.
5. You are entitled to SSS, PhilHealth and Pag-ibig as part of your benefits.

6. You are entitled for (5) Service Incentive Leaves (SIL) as part of your annual benefits. Next year
May 1, 2020 (up to 5 years) you will be given an additional one (1) day leave as part of your SIL.
Only (5) of SIL is convertible to cash if unused. There is no accumulation of leaves from the
present year to incoming year. You are also entitled to sick leave without pay.

V. WORKING HOURS
1. Working hours will be eight (8) hours a day, six (6) days a week which is equivalent to sixty (48)
hours a week.
2. The duty is from _______________(Monday to Saturday)
3. Overtime is given when there is a need and it is subject for approval by the Immediate Superior.
4. You are entitled for Regular and Special (non-working) Holidays.
VI. COMMITMENT
The appointment under this contract is a full-time appointment and the employee shall devote his full
commitment, energy, dedication and attention to the duties and responsibilities as stated in this contract.
The employee shall not at any time during the continuance of this contract be directly or indirectly
engaged, concerned or interested, whether for reward or otherwise, in any other business profession
without the explicit written consent of Company.
The employee will be subjected to the Disciplinary Procedure, Code of Conduct and Policies of the
Company.
