assessment result, you have the right to appeal. You
can submit your appeal to the TESDA provincial Office
where the Assessment Center and the Assessor were

Certificate. In requesting your NC that would be from
Monday-Friday 8:00AM to 5:00PM at TESDA Provincial
Office located at Dahican, Mati City.
Fill up the request form and attach the following:
original copy of you CARS, 1 passport size picture with
white background wearing collared garment and 2
documentary stamp. Then fill up the request for
payment and prepare Php 100.00, Php 50.00 for the
National Certificate then Php 50.00 for the
Certification, Authentication and Verification (CAV).

Candidate: I feel so nervous. I forgot the proper
sequence of demonstration and I almost pass out
during the assessment.
