The README states that nftlb can be easily integrated with your "preferred health checker" to build/maintain farms. The Debian package starts nftlb without a config file, so it seems to be strongly implied that this is the recommended way to use nftlb.
I've been unable to find anything that describes itself as a standalone health checker, though. What would one use with nftlb?

Hi! We use FarmGuardian that is included the ZEVENET ADC distribution. Also, you can use Nagios health checks and generate an easy script to disable/enable backends.
