Dr. Ailawadi, M.D., the Chair of Cardiac Surgery at Michigan Medicine, specializes in minimally invasive valve surgery as well as complex cardiac operations. This video shows step by step footage of a Coronary Artery Bypass Graft (CABG) in a complex patient. In this case, CABG was performed through a sternotomy (through the breast bone) using the internal thoracic artery and saphenous leg veins to bypass obstructed coronary arteries. In this video, Dr. Ailawadi will perform a triple vessel bypass (CABG) which has been shown to minimize the risk of future heart attack and help patients live longer in the setting of complex coronary artery disease.

The Difference Between a Cardiologist and a Cardiothoracic Surgeon
Cardiologists and cardiothoracic surgeons—both play an important role in heart health. "The difference between a cardiac surgeon and a cardiologist really involves the training and what they take care of," explained Dr. David Cervantes, a cardiothoracic surgeon at the Lee Health Shipley Cardiothoracic Center.
Cardiologists are trained to identify, diagnose and treat patients with heart conditions. "Cardiac surgeons are trained in surgery and we go through a long training process which allows us to take care of mechanical issues involved in the heart," he said.

Cardiologists and cardiothoracic surgeons—both play an important role in heart health. "The difference between a cardiac surgeon and a cardiologist really involves the training and what they take care of," explained Dr. David Cervantes, a cardiothoracic surgeon at the Lee Health Shipley Cardiothoracic Center.
Cardiologists are trained to identify, diagnose and treat patients with heart conditions. "Cardiac surgeons are trained in surgery and we go through a long training process which allows us to take care of mechanical issues involved in the heart," he said.
While both specialties work together to care for patients, doctors say there are certain diseases that should be managed by cardiologists and some that should be managed by cardiothoracic surgeons. "Cardiologists are the ones that traditionally implant stents, do heart caths, can implant pacemakers, and do arrhythmia procedures," said Dr. Cervantes.

Beginning with a midline sternal incision, the heart is connected to a perfusion machine which will take over the duties of the heart while the surgery takes place. Two different grafts are used to bypass the blocked coronary arteries: the internal thoracic artery from inside the chest wall, and the saphenous vein from the leg. After the procedure, the heart is shocked to restart its beating. A drainage tube is left at the incision site to drain away excess fluid. The animation continues to show two other types of approaches to a coronary artery bypass graft, off-pump bypass surgery and minimally invasive bypass surgery.
This is similar to the procedure performed on former president Bill Clinton and former California governor Arnold Schwarzenegger.
