Fault tree analysis (FTA) is a top down, deductivefailure analysis in which an undesired state of a system is analyzed using Boolean logic to combine a series of lower-level events. This analysis method is mainly used in the fields of safety engineering and reliability engineering to understand how systems can fail, to identify the best ways to reduce risk or to determine (or get a feeling for) event rates of a safety accident or a particular system level (functional) failure. FTA is used in the aerospace, nuclear power, chemical and process, pharmaceutical, petrochemical and other high-hazard industries; but is also used in fields as diverse as risk factor identification relating to social service system failure. FTA is also used in software engineering for debugging purposes and is closely related to cause-elimination technique used to detect bugs.
In aerospace, the more general term "system Failure Condition" is used for the "undesired state" / Top event of the fault tree. These conditions are classified by the severity of their effects. The most severe conditions require the most extensive fault tree analysis. These "system Failure Conditions" and their classification are often previously determined in the functional Hazard analysis.

Fault Tree Analysis is a type of Root Cause Analysis. It's method of diving deep into a problem or concern and working out its causes. It is another way of ensuring that problems are resolved and don't reoccur. We've looked at Fishbone Analysis in the past, and we now look at FTA in detail with an example.

Fault tree analysis with reliability calculation.
Fault tree analysis is a graphical tool to explore the causes of system level failures. It uses boolean logic to combine a series of lower level events and it is basically a top-down approach to identify the component level failures (basic event) that cause the system level failure (top event) to occur.

Fault Tree Analysis
Risk can be analyzed in one of two basic ways: inductively or deductively, that is either bottom-up or top-down. In a deductive analysis a system failure is postulated. The analyst then works backwards to deduce what combinations of events could have occurred for the system failure to have taken place (a detective solving a crime is thinking deductively). Fault tree analysis, the topic discussed in this section, is deductive. An inductive analysis works in the other direction. A single failure, such as a pump stopping or a valve closing at the wrong time, is postulated. The inductive approach then determines what impact the item failure could have on the overall system performance

Fault tree analysis is a graphical tool to explore the causes of system level failures. It uses boolean logic to combine a series of lower level events and it is basically a top-down approach to identify the component level failures (basic event) that cause the system level failure (top event) to occur.
