Bioidentical hormone replacement therapy (BHRT), also known as bioidentical hormone therapy or natural hormone therapy, is a term referring to the use of hormones that are identical on a molecular level with endogenous hormones in hormone replacement therapy. It may also be combined with blood and saliva testing of hormone levels, and the use of pharmacy compounding to obtain hormones in an effort to reach a targeted level of hormones in the body. A number of claims by some proponents of bioidentical hormone replacement therapy have not been definitively established through scientific testing. Specific hormones used in BHRT include estrone, estradiol, progesterone (which are available both in FDA-approved manufactured products and as pharmacy-compounded products), testosterone, dehydroepiandrosterone (both testosterone and dehydroepiandrosterone have some stringent limits placed on their availability and approval in Canada and the United States), and estriol (which is available in Europe but is not approved in Canada and the United States).
Hormone replacement therapy (HRT) is any form of hormone therapy wherein the patient, in the course of medical treatment, receives hormones, either to supplement a lack of naturally occurring hormones, or to substitute other hormones for naturally occurring hormones. Common forms of hormone replacement therapy include:
Hormone replacement therapy for menopause is based on the idea that the treatment may prevent discomfort caused by diminished circulating estrogen and progesteronehormones, or in the case of the surgically or prematurely menopausal, that it may prolong life and may reduce incidence of dementia. It involves the use of one or more of a group of medications designed to artificially boost hormone levels. The main types of hormones involved are estrogens, progesterone or progestins, and sometimes testosterone. It is often referred to as "treatment" rather than therapy.
Hormone replacement therapy of the female-to-male type is a type of hormone replacement therapy for transgender and transsexual people. It introduces hormones associated with the gender that the patient identifies with (notably testosterone for trans men and estrogen for trans women). Some intersex people also receive HRT, either starting in childhood to confirm the gender they were assigned at birth, or later in order to change the gender they were assigned at birth.
HRT causes the development of secondary sex characteristics. While HRT cannot undo the effects of a patient's first puberty, developing secondary sex characteristics associated with a different gender often allows the patient to "pass" or be seen as the gender they identify with, which causes significant social and psychological changes. Introducing synthetic hormones into the body impacts it at every level and many patients report changes in energy levels, mood, appetite, etc. The goal of HRT, and indeed all somatic treatments, is to provide patients with a more satisfying body that is more congruent with their true psychological gender identity.
Androgen replacement therapy (ART), often referred to as testosterone replacement therapy (TRT), is a class of hormone replacement therapy in which androgens, often testosterone, are replaced. ART is often prescribed to counter the effects of male hypogonadism. It typically involves the administration of testosterone, either by injection or by use of testosterone skin creams or gels. ART may also be prescribed to lessen the effects or delay the onset of normal male aging. However, this is controversial and is the subject of ongoing clinical trials, assessing the benefits and harms of its use in otherwise healthy older men. As men enter middle age they may notice changes caused by a relative decline in testosterone: fewer erections, fatigue, thinning skin, declining muscle mass and strength, more body fat. This dissatisfaction with the changes of aging has led to the development of the idea of androgen replacement therapy. Androgen replacement therapy is also used for men who have lost testicular function to disease, cancer, or other causes. For men who have had prostate cancer or at elevated risk, androgen replacement therapy remains controversial because some studies have shown that it increases the risk for prostate cancer; others refute that risk. Nonetheless, as of September 2014, testosterone replacement therapy has been under review for appropriateness and safety by the Food and Drug Administration due to the "potential for adverse cardiovascular outcomes".
