The Snoqualmie Indian Tribe is proud to offer Health and Wellness services to our members, their families and the surrounding community.
Tribal Member Services:
- Snoqualmie Behavioral Health Services (SBHS) – provides outpatient services including substance use disorder, mental health outpatient and intensive outpatient services via individual or group settings. Intakes, assessments and urinalysis testing is done on site, with additional care provided via telehealth and video conferencing platforms.
- Community Health Representative (CHR) – provides routine transportation to and from medical appointments within our five county Purchase/Referred Care (PRC) Delivery Area.
- Purchased and Referred Care (PRC) provides assistance with payments for healthcare, dental, vision, optometry, hearing aids and other services for eligible Tribal Members. The Tribal Assister can help Tribal Members with applying for other resources such as Apple Health or other health insurance.
- Elder Care Program – promotes the health and wellbeing of Snoqualmie Tribal Elders with nutritious meals, assistance with transportation and caregiver support.
Snoqualmie Behavioral Health Services (SBHS)
Snoqualmie Behavioral Health Services provides a wide spectrum of individual and group outpatient mental health services and substance use disorder treatment via in person, telephone, or tele-health through virtual means.
Mental Health
- Initial screening and intake to create tailored individual treatment plan
- Individual, couples, and family therapy
- Psycho-education, such as parenting training/classes, suicide awareness and prevention, trauma and PTSD awareness and treatment, including intergenerational trauma
- Group therapy, including Dialectical Behavioral Therapy groups for various problems/disorders
Substance Use Disorder
We believe addiction is a disorder, recovery is possible, and lives can be rebuilt.
We provide:
- Assessments
- Education
- Intensive Outpatient Treatment
- Referrals to Inpatient Treatment

Elder Care Program
Program Provides:
- Home Delivered Meals- these nutritious meals are provided in lieu of the Monthly Congregate Meals that have been suspended due to the pandemic.
- Transportation- provide transportation to and from medically necessary appointments for Tribal Elders that reside within the service delivery area (King, Snohomish, Pierce, Island and Mason counties).
- Caregiver Support- support to assist unpaid caregivers in providing care for Snoqualmie Tribal Elders.
- Nutritional Support-supplemental nutrition support for Tribal Elders.
- Support- assistance with accessing medical services and resources within the Snoqualmie Indian Tribe.

Non-Emergency Medical Transportation
The Snoqualmie Indian Tribe proudly offers Non-emergency transportation to and from your scheduled medical, dental, chemical dependency, and mental health appointments between the hours of 9am and 5pm Monday through Friday. A 72 hour advance notice is requested in order to help accommodate your scheduling needs. Transportation will be provided on a first-come, first-serve basis within the Snoqualmie Indian Tribe’s five county service areas (King, Snohomish, Pierce, Island, and Mason). Priority will be given to Tribal members with no means of transportation and those physically incapable of driving. Transportation services have been suspended due to the pandemic. We will provide updates as the program opens back up.
Medicaid Transportation Program
This program serves both Native and non–Native clients who are Medicaid enrolled and need transportation to and from scheduled medical, dental, mental health, or chemical dependency appointments between the hours of 9am and 5pm Monday through Friday. A 72 hour advance notice is requested in order to help accommodate your scheduling needs. Transportation will be provided on a first-come, first-serve basis within the Snoqualmie Indian Tribe’s five county service areas (King, Snohomish, Pierce, Island, and Mason). Purchased and Referred Care (PRC) - UPDATED
Purchased Referred Care provides assistance with payments for Physicians Services, Inpatient Hospital Services, Outpatient Hospital Services, including emergency room and outpatient surgery. Diagnostic Laboratory Services, Diagnostic and Therapeutic Radiology Services, Preventive Health Services, Optometry Services(vision), Physical Therapy, Chiropractic Care, Prescription Drugs and Dental for eligible Tribal Members. The Tribal Assister can also help Tribal Members with applying for other resources such as Apple Health or other health insurance. The PRC program year begins on October 1st and ends on September 30th.
