Introduction: Orthopaedic injuries constitute a major aspect of morbidity and mortality following... more Introduction: Orthopaedic injuries constitute a major aspect of morbidity and mortality following train accidents. The pattern of orthopaedic/musculoskeletal injuries sustained following these accidents has not been fully characterised. The main aim of this study is to describe the range of orthopaedic injuries reported in a major trauma centre and evaluate their management, as well as reporting mortality and amputation rates. Further aims are to identify the social and demographic background of the patients to suggest treatment and prevention strategies. Methods: This study is a retrospective observation of all clinical files of patients presented to Level 1 Trauma Centre in Cape Town, South Africa, as “train casualty” from January 2013 to July 2019, which were reviewed and evaluated. A total of 174 patients were included, of which 92 were orthopaedic referrals. The average age was 29 years, and 87% were male. A suite of subsea intervention case histories at the Bacchus oil field in the North Sea will demo... more A suite of subsea intervention case histories at the Bacchus oil field in the North Sea will demonstrate how one operator matured intervention planning to address well entry challenges using learnings gained over the course of successive jobs. This contributed to better management and mitigation of potential risks leading to slickline performance improvement for gas lift valve reconfiguration, the successful deployment of coiled tubing to clean out asphaltene deposits in a live subsea oil well from a monohull vessel and setting of a retrofit gas lift straddle to optimize and secure production. The paper outlines intervention asset selection, work programme development and risk mitigation measures related to subsea tree valve function issues and loss of full bore access caused by asphaltene and wax deposits. Light well intervention vessel and mobile rig operations using deployment methods including slickline, digital slickline, electric line and coiled tubing are described. The role of production technology work undertaken to better understand the nature of organic deposits in the wells and how that contributed to anticipating well access risks and inform intervention planning will be highlighted. These real field examples add to the knowledge base of well services and production technology challenges faced during subsea well intervention and highlights approaches to overcome them.

Thus, we investigated cerebrovascular and cardiovascular regulation in 23 adults ≥55 years of age, 10 diagnosed with ischemic stroke, and 13 age-matched healthy controls when sitting at rest and upon standing to compare differences of autonomic variables at ∼7 months (218 ± 41 days) poststroke.Arterial blood pressure via finger plethysmography, muscle-pump baroreflex via electromyography, heart rate variability via 3-lead ECG, and cerebral blood flow velocity via transcranial Doppler were analyzed while sitting for 5 minutes and then during quiet standing for 5 minutes.From the seated to standing position, the stroke group had significantly greater decline in the low frequency component of heart rate variability (164 [79] vs 25 [162] ms; P = 0.043). El disenador de pantallas resultante de este desarrollo, se divide en tres secciones: mimicos, tablas de senales y proyectos, los cuales poseen herramientas para su creacion, edicion y reproduccion. La seccion de mimicos permite crear pantallas que simulan los procesos para el tratamiento del agua, parcial o totalmente, consta de herramientas para la edicion de los elementos que se van agregando y un metodo que admite asociarle senales, con el fin de brindar cierto dinamismo. La segunda seccion se encarga de las tablas de senales, las cuales permite relacionar y organizar senales pertenecientes a un proceso, con el fin de mostrar los valores en campo. In order to identify the earliest genetic changes that precipitate species formation, it is usefu... more In order to identify the earliest genetic changes that precipitate species formation, it is useful to study genetic incompatibilities that cause only mild dysfunction when incompatible alleles are combined in an interpopulation hybrid. Such hybridization within the nematode species Caenorhabditis briggsae has been suggested to result in selection against certain combinations of nuclear and mitochondrial alleles, raising the possibility that mitochondrial-nuclear (mitonuclear) epistasis reduces hybrid fitness. To test this hypothesis, cytoplasmic-nuclear hybrids (cybrids) were created to purposefully disrupt any epistatic interactions. Experimental analysis of the cybrids suggests that mitonuclear discord can result in decreased fecundity, increased lipid content, and increased mitochondrial reactive oxygen species levels. Laparoscopic Roux-en-Y gastric bypass (LRYGB) provides sustained weight loss. However, short-term... more Laparoscopic Roux-en-Y gastric bypass (LRYGB) provides sustained weight loss. However, short-term studies have suggested that African Americans (AAs) are not as successful as Caucasians (CAs) after LRYGB. The present study was designed to test the hypothesis that at longer term follow-up AAs are just as successful as CAs after LRYGB. University hospital, United States. A nested case-control study designed to examine the effect of race as covariate in the long-term success of women undergoing LRYGB. The study matched 3 controls per case subject, and the final numbers for analyses were 78 case patients (AA) and 204 control patients (CA). Odds ratios (ORs) and 95% confidence intervals (CIs) were calculated using conditional logistic regression analysis. The 2 cohorts (N = 282) were well matched for age (AA 40.3±9.1 years versus CA 41.1±8.9 years), preoperative body mass index (AA 50.6±7.5 kg/m(2) versus CA 50.2±7.1 kg/m(2)), prevalence of type 2 diabetes (T2D) (AA 20.5% versus CA 21.1%), hypertension (AA 69.1% versus CA 52%), and sleep apnea (AA 35.9% versus CA 34.8%). In the AA group, the long-term curve for percentage of excess weight loss (EWL) was significantly (P<.001) lower than the CA group at any time-point. In the present model, diagnosis of T2D in the AA group (OR = 6.1 E8) significantly (P = .002) predicted adequate EWL at 3 years, after controlling for relevant confounders. Race significantly affected the long-term EWL at 3 years for patients undergoing LRYGB at the authors' institution. Future research should be directed at determining potential genetic reasons for these differences, including genes associated with T2D.

RESUMEN La profundidad de la base de la fuente magnética fue obtenida usando un método exponencia... more RESUMEN La profundidad de la base de la fuente magnética fue obtenida usando un método exponencial aplicado a los datos magnéticos del Golfo de México, principalmente sobre la corteza oceánica. El área de estudio fue subdividida por mallas de tres tamaños, una para ventanas de análisis de 100×100 km, otra para ventanas de análisis de 150×1150 km y otra para ventanas de análisis de 200×1200 km. Las mínimas profundidades obtenidas con las tres ventanas de análisis varían entre 14.0 y 17.0 km, y las máximas profundidades entre 35.0 y 37.5 km. En los tres casos en la parte central del golfo se observa una zona de profundidades someras. Las diferencias observadas entre las tres ventanas de análisis son las geometrías y tendencias de las profundidades. Los resultados de la ventana de 100×1100 km tienen mayor correspondencia con el modelo tectónico de corteza del Golfo de México. Las menores profundidades se observan sobre el límite de corteza oceánica-continental transicional, donde la profundidad del Moho sísmico es mayor.
