Step One: Please complete this form to initiate your request. This form should be used to request a disciplinary check, transfer request, or letter of good standing.
Step Two: Once you submit this electronic Google form, pleaseÂ EMAILÂ any supporting documents and/or forms that need to be signed or reviewed to [email protected] for completion.Â

***Note that the student section of the document/forms needs to be completed/signed by the student before sending it over to CSSC for our signature. Please check if the privacy section is signed and complete. Do not forget to sign the privacy release section.
