A Willoughby City Recreation Expenses Tax Levy question was on the November 6, 2012 election ballot in Lake County, which is in Ohio, where it was approved.

This question authorized the City of Willoughby to renew a property tax levy of 0.5 mills ($0.5 per $1,000 of assessed valuation) for five years in order to fund recreational services and expenses.[1]
