Scheduling an appointment with University ID Card Programs is strongly encouraged for groups of 4 or more who need an Iowa One Card or UI Health Care Badge.  If you know that you have a group of more than 4 individuals, please submit the following webform as soon as possible so that we can plan for your visit and make sure there are no scheduling conflicts.

You will receive e-mail confirmation from our office with additional instructions for your group. In the event we are unable to accommodate your preferred date/time, we will contact you with one or more alternate date/time options.
